Paracoccidioidomycosis is an invasive fungal infection. Involvement of the adrenal gland is a rare form of presentation that is associated with up to 10 to 15% of suprarenal insufficiencies and sometimes it can be the first clinical manifestation of the pathology. The objective of the study is to describe an atypical presentation of a patient with a diagnosis of bilateral tumor of the suprarenal gland, that was suspected of a tumor process of the suprarenal glands and the definitive diagnosis with ultrasound-guided puncture biopsy was adrenal paracocciodioidomycosis. Treatment is carried out with amphotericin B at a dose of 50 mg/day up to a cumulative dose of 1500 mg, with subsequent follow-up with itraconazole of 200 to 400 mg/day for a period of 6 to 12 months.
