Resumen

HERNANDEZ ROMERO, Rolando José et al. Pulmonary arterial hypertension secondary to dasatinib use. Rev. virtual Soc. Parag. Med. Int. [online]. 2026, vol.13, n.1, e13142617.  Epub 13-Mar-2026. ISSN 2312-3893.  https://doi.org/10.18004/rvspmi/2312-3893/2026.e13142617.

Pulmonary hypertension is a multifactorial disease whose diagnostic and therapeutic approach depends on the underlying cause. Among the less common aetiologies is drug-induced pulmonary arterial hypertension, including tyrosine kinase inhibitors used in haematological and oncological diseases.

We present the case of a 35-year-old female patient with a history of chronic myeloid leukaemia undergoing treatment with dasatinib, who consulted for progressive dyspnoea. Initial studies revealed severe pulmonary hypertension by transthoracic echocardiogram. Structural involvement of the left heart, pulmonary parenchymal disease, and pulmonary thromboembolism were ruled out. Right heart catheterization confirmed a haemodynamic pattern consistent with group 1 pulmonary arterial hypertension. In the absence of other identifiable aetiologies, a causal association with dasatinib use was established.

After discontinuation of the drug, significant clinical and haemodynamic improvement was observed, reinforcing the diagnosis of dasatinib-induced pulmonary arterial hypertension. This case highlights the importance of early recognition of this rare complication, the need for close cardiovascular monitoring in patients treated with tyrosine kinase inhibitors, and the value of a multidisciplinary approach to optimize prognosis.
