IBANEZ FRANCO, Elvis Javier et al. Genital lesions as an unusual presentation of chronic histoplasmosis in an immunocompetent patient. Rev. virtual Soc. Parag. Med. Int. Histoplasmosis is a systemic mycosis caused by Histoplasma capsulatum, which usually presents in the lungs. Skin involvement is uncommon, and genital involvement is exceptional, especially in patients without obvious immunosuppression.
We present the case of a 50-year-old immunocompetent female patient with chronic erythematous papular lesions in the genital region. Histopathology revealed yeast-like structures consistent with deep mycosis, and culture, together with identification by MALDI-TOF, confirmed infection with Histoplasma capsulatum. Tomographic studies demonstrated ground-glass pulmonary infiltrates and hepatosplenomegaly, consistent with chronic disseminated disease. The patient was treated with itraconazole for 12 months, achieving complete resolution of the lesions.
This case highlights the importance of considering histoplasmosis in the differential diagnosis of persistent genital lesions, even in patients without apparent immunosuppression, and emphasizes the value of specialized diagnostic methods for the early and accurate identification of this entity.
