Cytomegalovirus (CMV) is a virus of the Herpesviridae family whose primary infection in immunocompetent patients is usually asymptomatic or presents with mild symptoms similar to infectious mononucleosis. However, in cases of prolonged fever without a clear cause and with abnormal liver enzymes, it should be considered in the differential diagnosis.
We present the case of a 50-year-old immunocompetent patient with no relevant medical history who consulted for prolonged fever without other associated symptoms. Tests showed mild hepatomegaly, mesenteric adenitis, and elevated transaminases. Initially, viral serology was negative, but after four weeks, seroconversion was observed with positive IgM and IgG for CMV. The condition resolved spontaneously, confirming the diagnosis of CMV primary infection.
