ALK-negative anaplastic large cell lymphoma with primary cutaneous presentation is part of the spectrum of CD30+ lymphoproliferative diseases, along with lymphomatoid papulosis. It is a rare entity, accounting for a minimal proportion of non-Hodgkin lymphomas.
We present the case of a 63-year-old male patient who consulted for the presence of multiple exophytic skin lesions with poorly defined edges and a tendency to coalesce, some with ulcerations and a necrotic center, which were painless. The lesions involved the extremities, trunk, and lower limbs. The patient did not present with fever, weight loss, or other systemic symptoms. Laboratory and imaging studies were normal. A skin biopsy with immunohistochemistry was performed on one of the lesions on the left forearm, where a diagnosis of ALK-negative anaplastic large cell lymphoma with primary presentation in the skin was made.
