Paracoccidioidomycosis is a deep mycosis caused by the fungus Paracoccidioides. It is endemic in tropical and subtropical areas of Latin America. It is observed in rural workers and predominates in male patients. Diagnosis is made through histopathological and microbiological studies. The treatment of choice is itraconazole, but in selected cases, amphotericin B is used.
The case of a 58-year-old man is presented, who presents with lesions of 2 months of evolution in the mucosa of the right nasal ala that begins with ulceration, seropurulent discharge and pain, and another lesion on the hard palate that starts as an ulceration and subsequent fistulization, towards nasal passages. He has a history of mucosal leishmaniasis with nasal structural sequelae 20 years ago, so the initial suspicion was a recurrence of leishmaniasis. However, this infection was ruled out with directed studies and, instead, a diagnosis of paracoccidioidomycosis was made.
Treatment with itraconazole was started, with a good response in the control after the third month of treatment.
