Primary immunodeficiencies or inborn errors of immunity are disorders of genetic origin in which a cellular or humoral component of innate or adaptive immunity is altered, mainly generating a tendency to infections. There are 485 inborn errors of immunity distributed in ten categories, the tenth corresponds to phenocopies produced by somatic mutations or anti-cytokine antibodies. In the following case report, we present a patient who was admitted to the critical care unit due to septic shock of pulmonary origin secondary to phenocopy immunodeficiency to mycobacterial susceptibility. A review of the literature on this entity is made.
