An 84-year-old male patient with a history of endoscopic retrograde cholangiopancreatography (ERCP) and biliary stent placement for choledocholithiasis two years prior presented with a 15-day history of right upper quadrant abdominal pain, two episodes of fever reaching 38°C, and an unintentional weight loss of approximately 6 kg. Clinical evaluation revealed a colonic perforation secondary to migration of the biliary stent, an uncommon complication of ERCP. This case highlights the importance of careful long-term follow-up in patients with biliary stents, even years after the procedure, due to the risk of late migrations and severe complications such as perforation of adjacent organs.
