Neonatal lupus erythematosus (NLE) is a rare autoimmune disease in newborns, caused by the transplacental transfer of maternal autoantibodies. It is associated with characteristic cutaneous and extracutaneous manifestations.

We present three cases with cutaneous manifestations as the main symptom: one born to a mother previously diagnosed with Sjögren’s syndrome (SS), and two born to mothers without a known autoimmune disease at the time of delivery, later diagnosed with systemic lupus erythematosus (SLE) during the postpartum period-one of whom had a fatal outcome. All infants had a favorable clinical course.

This report highlights the importance of early diagnosis for proper neonatal management and, in some cases, for the detection of an underlying maternal autoimmune disease.
