Abstract
ALCARAZ BRITEZ, Adriana Guadalupe and NUNEZ ORREGO, Adriana Magalí. Extrapulmonary tuberculosis, in a young patient, about a case. An. Fac. Cienc. Méd. (Asunción) [online]. 2025, vol.58, n.1, pp.88-94. ISSN 1816-8949. https://doi.org/10.18004/anales/2025.058.01.88.
Introduction:
Mycobacterium tuberculosis is an intracellular bacillus, the agent of pulmonary and extrapulmonary tuberculosis. Extrapulmonary tuberculosis represents 20-25% of cases of tuberculous disease, a high index of suspicion is important for diagnosis.
Clinical case:
36-year-old male patient with symptoms of weakness of the left side (arm and leg), long-standing respiratory symptoms, weight loss and repeated urinary tract infections. Immunocompetent. Imaging results compatible with extrapulmonary and pulmonary manifestations of tuberculosis. GeneXpert positive in urine.
Discussion:
In the case presented, it was observed that the patient presents several positive data that coincide with the national epidemiological data, for example, he is male, he is within the predominant age as well as he is part of a risk population (time in prison).
Conclusion:
Misdiagnosis rates are often high, around 50-70%, even in TB-endemic countries, even more on inmunocompetentes patients. Therefore, clinical suspicion is a very important factor, as optimal treatment initiation is important. The treatment success rate is 84.7% and the recurrence rate is 0-14%.
