We present the case of a 79-year-old female patient with hypertension, dyslipidemia, and a history of upper gastrointestinal bleeding who is under follow-up by gastroenterology. Four months ago, she began experiencing progressive exertional angina symptoms. A coronary CT angiography was requested, which showed a significant lesion in a diagonal branch of good caliber. The decision was made to proceed with coronary angiography, which confirmed the presence of a first diagonal branch of adequate caliber and development, accompanied by a severe lesion in its proximal segment (85-90%). The remaining epicardial vessels did not exhibit significant lesions. The lesion was correctly predilatated, and a good opening of the vessel was achieved without any dissection. Considering the described comorbidities, it was decided to implant a Pantera Lux (Biotronik) drug-eluting balloon adjusted to the ostium of the first diagonal that covered the lesion, obtaining a very good final result.
