A clinical case of COMISA (combined insomnia and obstructive sleep apnea disorder) is presented, along with bruxism and periodic limb movements. The diagnostic and therapeutic complexity of this condition is highlighted through the use of clinical tools and polysomnography. Findings show sleep fragmentation, severe hypoxia, and abnormal muscle activity. A multidisciplinary management approach is proposed, including weight loss, CPAP, and cognitive-behavioral therapy. The high prevalence of sleep disorders in Latin American countries is emphasized, as well as the bidirectional relationship between insomnia and OSA, and the importance of identifying comorbidities. Finally, the need for a multidisciplinary approach is underscored, as these disorders are associated with cardiovascular risk, depression, and reduced productivity.
