The identification of transfusion-transmissible infections is essential to ensure the safety of blood components administered to patients. This study aimed to determine the seropositivity rate of major infectious markers among donors at a referral hospital. A cross-sectional, observational, and descriptive study was conducted throughout 2024, analyzing the serological results of all donors who met the selection criteria established by the Pan American Health Organization. A total of 29,313 donations were recorded, of which 1,979 (6.75%) tested reactive for at least one marker. The average age was 38 years; most donors were male (77.78%) and replacement donors (90%). The most frequent markers were: syphilis (3.61%), total antibodies against hepatitis B core antigen (1.24%), Chagas disease (0.89%), hepatitis C (0.35%), and human T-lymphotropic viruses types 1 and 2 (0.28%). Systematic screening in blood banks is essential to ensure the quality and safety of transfusion practices.
