En Paraguay, la evidencia sobre características clínicas y hallazgos diagnósticos en el sistema público de salud es limitada, lo que dificulta la optimización de protocolos de manejo.  Objetivo:  Describir los hallazgos histeroscópicos e histopatológicos en pacientes con sangrado uterino postmenopáusico atendidas en el Hospital Nacional de Itauguá, Paraguay, durante enero de 2019 a abril de 2022.  Materiales y Métodos:  Estudio transversal descriptivo. Se incluyeron 160 pacientes postmenopáusicas con sangrado uterino sometidas a histeroscopia diagnóstica. Se analizaron características demográficas, factores de riesgo endometrial, hallazgos histeroscópicos e histopatológicos mediante estadística descriptiva.  Resultados:  La edad media fue 60±8,3 años. Los principales factores de riesgo identificados fueron obesidad (71,9%) y diabetes mellitus (22,5%). Los hallazgos histeroscópicos más frecuentes correspondieron a pólipos endometriales (43,8%) y atrofia endometrial (31,3%). El análisis histopatológico reveló pólipos benignos (41,9%) y adenocarcinoma endometrial (10,0%). La distribución según malignidad mostró: lesiones benignas (86,4%), malignas (10,0%) y lesiones precursoras (3,6%).  Conclusiones: Los pólipos endometriales constituyen el hallazgo predominante en sangrado uterino postmenopáusico. La frecuencia significativa de adenocarcinoma (10,0%) confirma la necesidad imperativa de evaluación histopatológica sistemática para detección temprana de malignidad endometrial en el sistema público paraguayo, justificando el mantenimiento de protocolos de estudio rigurosos en esta población de riesgo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Background:  Postmenopausal uterine bleeding constitutes a diagnostic emergency requiring urgent ruling out of endometrial malignancy. In Paraguay, evidence regarding clinical characteristics and diagnostic findings in the public health system is limited, hindering the optimization of management protocols.  Objective:  To describe hysteroscopic and histopathological findings in patients with postmenopausal uterine bleeding treated at Itauguá National Hospital, Paraguay, from January 2019 to April 2022.  Materials and Methods:  Observational, descriptive, cross-sectional study. A total of 160 postmenopausal patients with uterine bleeding who underwent diagnostic hysteroscopy were included. Demographic characteristics, endometrial risk factors, hysteroscopic and histopathological findings were analyzed using descriptive statistics.  Results:  Mean age was 60±8.3 years. The main risk factors identified were obesity (71.9%) and diabetes mellitus (22.5%). The most frequent hysteroscopic findings corresponded to endometrial polyps (43.8%) and endometrial atrophy (31.3%). Histopathological analysis revealed benign polyps (41.9%) and endometrial adenocarcinoma (10.0%). Distribution according to malignancy showed: benign lesions (86.4%), malignant lesions (10.0%), and precursor lesions (3.6%).  Conclusions:  Endometrial polyps constitute the predominant finding in postmenopausal uterine bleeding.
