Estos pacientes tienen mayor riesgo de desarrollar infección urinaria (ITU) por instrumentaciones frecuentes, sobre distensión vesical, presiones vesicales aumentadas, reflujo vesicoureteral y presencia de residuo pos miccional. La frecuencia de bacteriuria asintomática varía de 42 a 76 %, y la incidencia estimada de ITU es de 2.5 episodios por año. Los gérmenes más frecuentemente involucrados son la E. coli y la Klebsiella pneumoniae, pero también se aíslan Pseudomonas aeruginosa, Serratia, Proteus, Acinetobacter, Enterococcus faecalis y hongos. Se considera que un paciente con vejiga neurogénica tiene ITU, si reúne los siguientes criterios: urocultivo positivo para un único gérmen con &gt;100.000 UFC/ml, asociado a sedimento urinario patológico y clínica compatible con 2 o más de los siguientes signos o síntomas: Fiebre, escalofríos, dolor abdominal, dolor lumbar, incontinencia nueva o empeoramiento de la misma y hematuria. El cateterismo limpio intermitente (CIL) se utiliza para disminuir la presión intravesical y se asocia con la administración de agentes anticolinérgicos, para tratar las contracciones inestables del detrusor, con el fin de preservar la función renal. Para el tratamiento de las ITU, se aconseja la utilización de antibióticos de bajo espectro por el menor tiempo posible. No se aconseja el tratamiento de la bacteriuria asintomática.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT Myelomeningocele is the most common cause of neurogenic bladder in children. These patients are at increased risk of developing urinary tract infection (UTI) by frequent instrumentations on bladder distention, increased bladder pressures, presence of vesicoureteral reflux and after micturition residue. Asymptomatic bacteriuria frequency varies from 42 to 76%, and the incidence is estimated ITU 2.5 episodes per year. Germs most frequently involved are E. coli and Klebsiella pneumoniae, but Pseudomonas aeruginosa, Serratia, Proteus, Acinetobacter, Enterococcus faecalis and also isolated fungi. It is considered that a patient with neurogenic bladder has ITU, if it meets the following criteria: positive urine culture for a single germ with &gt; 100.000 CFU / ml, associated with pathological urinary sediment and clinical compatible with 2 or more of the following signs or symptoms: fever , chills, abdominal pain, back pain, new or worsening incontinence and hematuria same. The clean intermittent catheter (CIL) is used to decrease bladder pressure and associated with the administration of anticholinergic agents, for treating unstable detrusor contractions, in order to preserve renal function. For treatment of UTI, the use of low spectrum antibiotics for the shortest time possible is advised. treatment of asymptomatic bacteriuria is not recommended.]]></p></abstract>
