Dicha l&iacute;nea de investigaci&oacute;n es desarrollada a partir  del a&ntilde;o 2.006 en el Departamento de Salud P&uacute;blica del Instituto de  Investigaciones en Ciencias de la   Salud (IICS) de la Universidad Nacional  de Asunci&oacute;n (UNA) (1-13). El c&aacute;ncer de cuello uterino es el tercer tumor  maligno m&aacute;s frecuente en mujeres a nivel mundial y el segundo m&aacute;s frecuente  entre las mujeres en Latino Am&eacute;rica. Aproximadamente, el 80% de los casos de  c&aacute;ncer de cuello uterino detectados a nivel mundial ocurren en pa&iacute;ses en v&iacute;as  de desarrollo. Paraguay se encuentra entre los pa&iacute;ses con mayor incidencia y  mortalidad de c&aacute;ncer de cuello uterino, en donde se diagnostican m&aacute;s de 2 casos  de c&aacute;ncer por d&iacute;a y se producen m&aacute;s de 1 muerte por d&iacute;a por este mal (14). La  magnitud de esta afecci&oacute;n acarrea un verdadero problema social y de salud  p&uacute;blica para el pa&iacute;s; considerando que el c&aacute;ncer de cuello uterino es 100%  prevenible y curable.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Entre  los hallazgos m&aacute;s importantes que observamos en los trabajos desarrollados a  nivel local se encuentra la detecci&oacute;n de una alta frecuencia de virus de  papiloma humano de alto riesgo oncog&eacute;nico (VPH-AR) en mujeres sin y con  lesiones precancerosas de cuello uterino (1,2). Entre estos el VPH 16 ocup&oacute; el  primer lugar en frecuencia, lo cual es relevante debido a que una infecci&oacute;n  persistente por VPH-16 confiere mayor riesgo de c&aacute;ncer de cuello uterino  invasivo comparada a una infecci&oacute;n con otros tipos virales de alto riesgo  oncog&eacute;nico. M&aacute;s de la mitad de los casos de c&aacute;ncer de cuello uterino en mujeres  paraguayas se debe al HPV 16. Kasamatsu et al., 2.012 y Mendoza et al., 2011  han detectado una frecuencia de HPV 16 del 68,2% y 64,6%, respectivamente (3).</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las investigaciones sobre la diversidad del  genoma del VPH-16 han identificado 5 ramas filogen&eacute;ticas (como; europeos, E;  asi&aacute;ticos, As; African tipo 1, Af-1; African tipo 2, Af-2; y  asi&aacute;tico-americano, AA) que muestran a nivel de nucle&oacute;tidos m&aacute;s de 98% de  homolog&iacute;a en E6 y L1, y m&aacute;s del 95% en la regi&oacute;n de control (LCR). Estudios  epidemiol&oacute;gicos han observado que las variantes no europeas del VPH-16,  espec&iacute;ficamente las variantes de origen asi&aacute;tico, se asocian con lesiones  cervicales y el c&aacute;ncer (4). </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La regi&oacute;n LCR es una regi&oacute;n no codificante  de 850 pb (~ 12% del genoma viral) que incluye un fragmento responsable de  regular la transcripci&oacute;n de genes de VPH-16 por una compleja interacci&oacute;n de  muchos factores celulares y virales que se unen a esta regi&oacute;n de regulaci&oacute;n.  Sustituciones de nucle&oacute;tidos en esta regi&oacute;n dentro de sitios de uni&oacute;n de  factores de transcripci&oacute;n dan como resultado una variaci&oacute;n en la actividad del  promotor de oncoproteinas virales (4).</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Dado  que las variantes de VPH 16 poseen diferente potencial oncog&eacute;nico y que el  an&aacute;lisis de la variabilidad gen&oacute;mica de LCR aporta datos que contribuyen a  comprender el por qu&eacute; de esta diferencia oncog&eacute;nica, el objetivo del presente  estudio fue caracterizar la variabilidad gen&eacute;tica de la regi&oacute;n LCR del VPH-16  aislados a partir de mujeres paraguayas con y sin lesiones cervicales (4).</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La mayor&iacute;a de las variantes de VPH-16  perteneci&oacute; a la rama europea (82%). Dos aislamientos produjeron nuevas  variantes paraguayas de la rama europea nombradas new PYa y new PYb. Variantes  no europeas, como la africana de tipo 1 (1,5%) y asi&aacute;tico-americana (16.5%) se  detectaron s&oacute;lo entre las mujeres con lesiones cervicales (4).</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La variante new-Pya se detect&oacute; en 1 mujer  con LSIL y conten&iacute;a una sustituci&oacute;n de nucle&oacute;tidos (A7752C) en el sitio de  uni&oacute;n para el factor de transcripci&oacute;n YY1. En las variantes no europeas se  observaron al menos 6 sustituciones de nucle&oacute;tidos adyacentes o ubicados dentro  de sitios de uni&oacute;n de factores de transcripci&oacute;n relacionados con la producci&oacute;n  de oncoproteinas virales. Esto podr&iacute;a explicar en parte que estas variantes  hayan sido detectadas solo en mujeres con lesiones (4).</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Adem&aacute;s, se observaron variantes previamente  descriptas por Ho et al., 1.993 como IND8, AM6, AN12, y B14. Las variantes AN12  y AM6 descrito por Ho et al. fueron detectadas por primera vez en las tribus  nativas de Am&eacute;rica del Norte y del Sur, respectivamente. En particular, estas  variantes a&uacute;n no se han identificado en la poblaci&oacute;n general de Brasil o  Argentina. Los datos actuales pueden contribuir a aumentar nuestro conocimiento  del origen de la infecci&oacute;n por VPH en Paraguay; la epidemiolog&iacute;a molecular de  la infecci&oacute;n por VPH en nuestro pa&iacute;s parece ser compleja y diferir ligeramente  de la del resto de la regi&oacute;n (4). </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Finalmente, aunque las variantes no  europeos se consideran m&aacute;s oncog&eacute;nicas, en el presente estudio se detectaron  variantes europeas del VPH-16 entre las mujeres con y sin lesiones cervicales.  Estos resultados enfatizan el hecho de que todas las ramas de variantes del  VPH-16 est&aacute;n relacionadas con el desarrollo de pre-c&aacute;ncer y c&aacute;ncer invasivo,  aunque factores adicionales parecen tener un papel en la carcinog&eacute;nesis  cervical (4).</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Actualmente, el equipo de investigaci&oacute;n del Departamento de Salud  P&uacute;blica, IICS, UNA (Dres. Laura Mendoza, Pamela Mongel&oacute;s, Graciela Gim&eacute;nez,  Mar&iacute;a Isabel Rodr&iacute;guez, Amalia Castro, Adriana Valenzuela, F&aacute;tima Cardozo,  Elena Kasamatsu, Malvina Paez) se encuentra trabajando en la caracterizaci&oacute;n  molecular de VPH en poblaciones de riesgo en muestras de cuello uterino y de  otras localizaciones. Desde el a&ntilde;o 2.013 el equipo est&aacute; formando parte de un  trabajo de investigaci&oacute;n internacional titulado "Estudio multic&eacute;ntrico de  tamizaje y triage de c&aacute;ncer de cuello uterino con pruebas del Virus de Papiloma  Humano (ESTAMPA)&quot;, coordinado por la Agencia Internacional  de Investigaci&oacute;n sobre C&aacute;ncer (IARC), Organizaci&oacute;n Mundial de la Salud (OMS), Francia. Este  estudio tiene como objetivo evaluar estrategias de implementaci&oacute;n de programas  organizados de tamizaje basados en pruebas de HPV e investigar el desempe&ntilde;o de  las diferentes t&eacute;cnicas emergentes en el triage de c&aacute;ncer de cuello.</font> </p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>REFERENCIAS BIBLIOGR&Aacute;FICAS</b></font></p>     ]]></body>
