Thyrotoxic Periodic Paralysis is (TPP) is a sporadic condition that presents with transient and recurrent muscle weakness and severe hypokalemia in patients with thyrotoxicosis. This case is presented since it is a very rare complication of hyperthyroidism, even more so when it is presented as the debut of Graves Basedow disease. The objective is to present the clinical characteristics, treatment and evolution of a patient with thyrotoxic periodic paralysis. The patient started symptoms of Graves Basedow disease, with periodic hypokalemic paralysis, which improved rapidly with the correction of intravenous potassium deficiency (K+). Pathologies that present with upper and lower limb weakness must be correctly identified, since they can easily be confused with others diseases that need different diagnosis and more aggressive treatment.
