The clinical spectrum of vasculitis is highly dependent on damaged blood vessels and affected organs. They all share common symptoms such as constitutional symptoms and the presence of anti-neutrophil cytoplasmic antibody. Microscopic polyangiitis is the most frequent vasculitis in the elderly population. The case of a 76-year-old patient with a history of recurrent alteration of the urinary sediment, chronic anemia, muscle weakness, gait disorders, non-productive wet cough, with frequent hospitalizations of genitourinary or respiratory origin is presented. Laboratory profiles, viral serologies, rheumatological profiles including ANCA are requested in case of clinical suspicion of vasculitis and evaluations by specialists. In a clinical picture suggestive of renal, pulmonary, and neurological involvement, documented by 24-hour proteinuria and creatinine clearance, spirometry, chest tacar, electromyography, and with positive ANCA, treatment with corticosteroids and immunosuppressants was started. Despite the treatment carried out and the slight improvement in the clinical picture at the beginning, the late diagnosis added to poor prognosis factors and the abandonment of the treatment, lead to the death of the patient.
