Progressive multifocal leukoencephalopathy is a demyelinating disease of the central nervous system caused by a virus of the Polyomavirus genus. The clinical manifestations can be motor, sensory or cognitive.
We present the case of a 32-year-old male patient with a 24-hour evolution of weakness in the left upper and lower limb that began insidiously and progressively, accompanied by dysarthria and confusion.
Due to suspicion of cerebral vasculitis versus demyelinating disease, corticosteroid boluses are started, which improves weakness. Laboratory studies are requested in which AIDS is confirmed. Gadolinium magnetic resonance imaging shows lesions compatible with progressive multifocal leukoencephalopathy. Antiretroviral treatment is started and he is discharged without other complications.
