Sarcoidosis is an idiopathic systemic inflammatory disease that mainly affects the lungs. Extrapulmonary manifestations occur in approximately 30% of patients. The most common sites of extrapulmonary disease include the skin, eyeballs, reticuloendothelial system, musculoskeletal system, exocrine glands, heart, kidneys, and central nervous system. We present a clinical case of sarcoidosis with involvement of peripheral lymph nodes, renal and cutaneous alterations without pulmonary involvement, which represents a diagnostic challenge due to the absence of the typical characteristics of this pathology.
