Abstract

Introduction:

Colistin is an effective antibiotic to treat infections caused by multiresistant Gram-negative bacteria but tends to produce acute renal injury.

Objectives:

To determine the frequency of acute renal injury in adult patients treated with colistin and to identify the presence of associated comorbid conditions.

Methodology:

Retrospective observational study of patients treated with colistin in the National Hospital, Paraguay in 2016-2017.

Results:

Eighty patients were studied and 64% of them were male. The mean age was 50±17 years. The prevalence of acute renal injury was 70%. According to the RIFLE criteria there was an injury in 45% of the cases, risk of injury in 25% and no risk/no injury in 30%. The most common comorbid conditions were hypertension (61%) and diabetes mellitus (31%). The most frequently isolated germs were Acinetobacter baumannii (57%), Pseudomonas aeruginosa (24%) and Klebsiella pneumoniae (19%). The most commonly used antibiotics in combination with colistin were vancomycin (46%) and carbapenems (43%).

Conclusions:

The prevalence of acute renal injury was 70%. According to the RIFLE criteria, the injury predominated. The most common comorbid conditions were hypertension and diabetes mellitus.
