The liver is the largest organ in the abdomen, leaving it highly vulnerable during abdominal trauma. Hepatic hematoma by definition is injury to the liver due to blunt or penetrating trauma, both of the abdomen and the lower part of the right thorax. The evolution in the diagnosis of liver trauma has achieved a decrease in mortality in recent years; it is generally very rare with few specific symptoms. Currently, management tends to be more and more conservative, leaving surgery for specific cases. Hemodynamic stability is the most important factor that influences the choice of the appropriate patient. The main cause of non-operative treatment failure is persistent bleeding. We present the case of a 66-year-old male, who came to the service with a history of falling from his own height and present wit pain in the right upper quadrant.
