Choledocholithiasis or presence of calculus in the extra hepatic main bile duct in the majority of cases is endoscopic resolution by endoscopic retrograde cholangiography (ERCP); but in certain cases, the surgical resolution is indicated due to the disproportion between the calculus/choledocho and other factors that make endoscopic resolution difficult or impossible. The objective of this work is to present the laparoscopic video resolution of a patient with a diagnosis of bile duct stones, with a history of cholecystectomy 12 years earlier. This is a 69-year-old female patient with epigastric pain, 30 days of evolution, accompanied by nausea, vomiting, jaundice and choluria. At hospital admission, she presented moderate acute cholangitis and a large bile duct stone, corroborated with ERCP, due to disproportion of the size of the calculus and common bile duct. Surgicallaparoscopic treatment was performed.
