Thoracic endometriosis is the presence of functional ectopic endometrial tissue within the thoracic cavity. Catamenial pneumothorax is the spontaneous pneumothorax that occurs near menstruation. We present the case of a 35-year-old woman with a diagnosis of right recurrent pneumothorax. Thorax CT shows bilateral apical bullae. In the thoracoscopy, the bullae are resected and biopsy of the violaceous nodules in the parietal pleura and diaphragm resulting in endometriosis. She is discharged in the hormonal treatment plan.
