Chronic xanthogranulomatous cholecystitis is uncommon.It is characterized by dense squamous cell infiltrates accompanied by fibrosis.It has been proposed that rupture and intramural infiltration of bile and mucin may be the cause of the characteristic xanthogranulomatous reaction. Its appearance sometimes causes confusion with a malignancy.May cause infiltration to other structures, fistulas or perforation. We present the case of a 75-year-old man who was admitted with a diagnosis of an icteric obstructive lithiasic syndrome, who underwent the failure of two ERCP cholecystectomy with CPO, whose biopsy result reported chronic xanthogranulomatous cholecystitis. Good evolution without complications.
