At the level of the midline of the anterior wall of the abdomen there are commonly certain wall defects, they are: the umbilical hernias, the epigastric hernias and the diastasis of the recti. The technique of MILOS (Mini and Less Open Sublay) allows a repair of these defects with the placement of a prosthetic material in the pre-peritoneal muscle retro space (sub lay) with good results. With the aim of assessing the results of the application of this technique is presented this prospective descriptive observational study on adult patients of both sexes, carriers of primary defects of the midline undergoing surgical treatment with MILOS technique from March to November 2017 in the Abdominal Wall Surgery Unit of the Second Chair of Surgical Clinic of the Clínicas Hospital of the Faculty of Medical Sciences, National University of Asunción. 12 patients were intervened, carrying 13 defects of the midline, average age of 48 years, predominance of the female sex and an average of 3 cm parietal defect. In 7 patients, the open MILOS technique was performed and in 5 the MILOS technique with endoscopic vision. The average surgical time was 144 minutes and the hospital stay was 1.8 days. In the immediate postoperative period, 9 patients showed mild pain and 4 moderate pain according to the Eva scale. Two patients presented local complications. The MILOS technique allows the repair of this type of defects with satisfactory with satisfactory results and minimal morbidity.
