Abstract

18 years old teen with 38 weeks gestation, was seen in her community with a history of 3 days of progressive dispnea. Accom-panied by lower limb edema and high pressure values. Developed episode of generalized seizures is referred to the National Hospital. Admitted the in Department of Obstetrics and Gynecology and eclampsia was diagnosed. Cesarean section was indicated, and in the inmediate postoperative was admitted to the intensive care unit. The electrocardiogram shows ST-segment depression in anterior, lateral, septal and apical walls, elevation of cardiac troponin I. Acute extensive myocardial infarction was diagnosed. Trans-thoracic echocardiography shows anterior, septal and apical akinesia and hypokinesia of the remaining cardiac walls with an ejection fraction of 28-32%. The patient condition improved. A week later the electrocardiogram is normal, control transthoracic echocardiography revealed persistent changes in cardiac of VI, motility with improvement of Left Ventricle ejection fraction (48%). One month later, echocardiography shows normal systolic function with ejection fraction 60%, no with residual abnormal motility.
