ABSTRACT Introduction: Hydatid cyst or echinococcosis is a zoonosis caused by the larval stage of the cestode Echinococcus granulosus, E. vogeli, E. multilocularis. Humans acquire hydatic cyst ingesting viable parasite eggs contained in their food. The most affected organs are the liver and lung, renal localization is rare. Case report: 55 year old woman, teacher, from rural area, presented with a two months history of right flank pain. Physical examination revealed right flank pain. Renal ultrasound revealed a right parapielic cyst with signs of compression. She underwent ablation of the cyst. Biopsy revealed hydatid cyst and hydatid serology was positive. After surgery she received allbendazole 400 mg for four weeks, and the lesion went back. Discussion: In many cases this infection is detected only in a routine imaging studies. When symptoms appear, they appear as a mass effect produced by enlargement of the cyst as was in this case . The renal location is rare and should be considered when hydatid cyst is in the differential diagnosis of renal cysts.
