Abstract Pandemic Influenza H1N1 virus affected the population of Paraguay in the first week of July 2009. It was accompanied by an unusual outbreak of community-acquired pneumonia. Objectives: To describe the demographic and clinical characteristics of an outbreak of pneumonia in the community during the pandemic of H1N1 influenza. Methods: observational, descriptive and prospective study, involving 97 adult subjects, clinical carriers of influenza-like illness accompanied by acute community-acquired pneumonia, hospitalized in the Department of Internal Medicine (National Hospital, Itauguá) during the months of July and August 2009. Results: predominance of females (66%), mean age 42±17 years. The most frequent comorbidities were: arterial hypertension (39,2%), age over 60 years (17,5%), pregnancy (14,4%). The clinical and laboratory characteristics were: bilateral pulmonary involvement (95,4%), leukopenia, hypertransaminemia (14,4%), myocarditis (22,2%). On chest radiographs predominated bilateral interstitial infiltrate (58,5%). Most of the blood cultures were negative. The mortality rate was 2,1%. Conclusions: this outbreak identified hihh morbidity but low mortality.
