Opportunistic germs can cause serious infections in immunosuppressed patients, such as patients with the human immunodeficiency virus. We present the case of a 35-year-old woman, a cardiac patient with a mechanical aortic valve prosthesis, anticoagulated, who presented with an 8-day history of dry cough, loss of appetite and pale skin. 3 days before admission, a fever of 38.5 ° was added without a predominance of hours, accompanied by chills that partially subsided with antipyretics and physical means. It accompanies the picture generalized weakness and respiratory distress at moderate efforts. He reported a similar picture 17 months ago, with a diagnosis of bacterial aortic endocarditis to Streptococcus gallolyticus spp, which required aortic valve replacement with a mechanical prosthesis. A video colonoscopy is performed where a vegetating tumor is observed at the level of the ascending colon and whose pathological anatomy reports a villous adenoma with Intramucosal adenocarcinoma foci with free margins.
