Abstract

Paracoccidioidomycosis is a systemic mycosis of great importance for the countries of Latin America. When it is not diagnosed and treated timely it can progress to severe disseminated forms. Isolated cases of single or multiple bone involvement are described, usually in the disseminated form.

Clinical case: Male, aged 73, bricklayer, from San Antonio - Pte. Hayes, a 3 months evolution, ulcerated lesion in the external malleolar region of the left ankle, starts as a single lesion, approximately 1 cm in diameter, then increases size and small lesion appears with similar feature in neighborhood. One week before, pain and serous discharge at the level of the lesions for which he consults. Scarification of the lesion is performed: exoesporulated mycotic spores compatible with Paracoccidioides sp. and hospitalization is decided. In the hard palate, an ulcerated granulomatous lesion of 0.5 x 1 cm is observed. In generalized pallor skin and ulcerated lesions in the external malleolar region of the left foot, number of 2, rounded, net borders, with serous secretion and painful perilesional swelling. Escharification of ulcer on the palate: Paracoccidioides sp. Serology for Paracoccidioides brazilienzis: positive 1 / 2. Chest CT: perihilar and bibasal parenchymal infiltrate with cavitary images. TAC MII: osteolytic lesion in a fibula sac. Received 1.5 gr. accumulation of AmB deoxycholate with good evolution and external with Cotrimoxazole with a treatment plan of at least 2 years.
