Abstract

Background: Dengue fever in its clinical expression varies from mild to severe, whose prevalence can vary in each epidemic. Aim: To characterize the clinical behavior of pediatric dengue cases hospitalized during the outbreaks of 2007, 2009 and 2010 in a reference hospital in Paraguay. Materials and Methods: A retrospective descriptive study based on data from medical records of pts admitted to the IMT in the period January 2007 to July 2010 with a discharge diagnosis of dengue. We analyzed demographic, clinical, laboratory, and complications. Results: During the study period 147 children were hospitalized with a diagnosis of Dengue, corresponding to 98 pts (67%) to 2007, 28 pts (19%) by 2009 and 21 (14%) by 2010. The predominant serotype in 2007 was DENV3 and in 2010 the DENV1. The mean age of pts was 13.2 ± 3.8 years, corresponding to 2% to the age group <5 years, 37% to 61% 5-12 years and 13-17 years group (p <0,05). 36 pts (24%) met criteria for severe form of the classification DENCO, confirming shock in 21 pts (14%). When comparing the clinical performance in different years, the incidence of visceral complications (encephalitis, myocarditis, hepatitis) was higher in the epidemic 2007 (24/98, 24%) vs 5 / 49 (10%) in 2009-2010 (p <0,05) and the frequency of thrombocytopenia <50.000 (29% vs 12%, p=0,01). Series mortality was low (1,4%). Conclusions: This study reveals the characteristics of the epidemic in terms of severity, and the high incidence of visceral complications in relation to serotype DENV3.
