Resumen

BENITEZ, Ana Alicia María et al. Funcional and aesthetic rhinoplasty, our experience in the Otorhinolaryngology and Head and Neck Surgery Service. An. Fac. Cienc. Méd. (Asunción) [online]. 2020, vol.53, n.2, pp.17-24. ISSN 1816-8949.  https://doi.org/10.18004/anales/2020.053.02.17.

Introduction:

In the region there are several academic environment centers that perform rhinoplasty, and the experience of the otorhinolaryngologist is increasingly enough in terms of nasal aesthetics. Due to the high prevalence of associated aesthetic and functional disorders, it is necessary that the professional is trained to solve both problems, even in the same surgical procedure

Objectives:

To describe the approaches and techniques used in rhinoplasty surgery in our service.

Materials and methods:

Observational, descriptive study with analytical components, transversal, retrospective, non-probability sampling of consecutive cases, of patients with altered aesthetics and nasal functionality who attended the Otorhinolaryngology department of the Hospital de Clínicas, San Lorenzo since January 2016 to June 2019.

Results:

76 rhinoplasties were performed in the service, 76.3% of them also required septoplasty and 61.8% required turbinectomy. 68% were female. The mean age was 25.21 ± 8.79 years. 47.36% of the reasons for consultation were due to nasal obstruction, followed by both obstructive and aesthetic symptoms in 42.1%; and 10.52% purely aesthetic. 40.78% of the patients in the study reported a history of nasal trauma. Surgery was performed by open approach in 71.05% of cases and in 28.95% by closed approach. The most frequent procedures were the placement of Strut Columellar (89.47%), Back Resection (78.94%) and Osteotomy (72.36%). The most frequent complication was epistaxis in 5.26%, all of them with a history of osteotomy. 100% of the patients with obstruction reported improvement, and in 3 there was persistence of slight laterorrinia.

Conclusion:

76 Rhinoplasties were performed from January 2016 to June 2019, most also required septoplasty and turbinectomy. The average age was 25.21 years, and most of the female sex. Cosmetic surgery in the male population was more frequent in patients with a traumatic history (p <0.05 in the xi-square test). Most of the patients had as their main drawback functionality rather than aesthetics. The most frequent approach was by open approach, and the procedures most frequently were the Columellar Strut, the Resection of the Dorsum and the Osteotomy. The most frequent complication was epistaxis, which in most cases was resolved with topical methods. All patients with obstructive symptoms reported functional improvement, laterorrinia was the most difficult alteration to correct.
