Bowen's disease is an intraepidermal squamous cell carcinoma with invasive potential that is seen more frequently in people older than 60 years. It usually occurs as a slow-growing erythematous plaque in photo-exposed areas.

We present the case of a 65-year-old woman with squamous cell carcinoma in situ pagetoid variety located in the left breast, in which immunohistochemistry was decisive in order to make the final diagnosis.
