ABSTRACT Introduction: The Rhinophyma is a chronic inflammation of the tissues lining the nose that may occur with exophytic growth and / or telangiectases. It is considered a rare subtype of rosacea in advanced stage causing sequelae functional, aesthetic and psychological. The treatment is surgical. Objective: To report a case of rhinophyma with ventilatory functional impotence. Report a case: The authors report the case of an adult patient with rhinophyma, with functional and aesthetic consequences of consideration, treated surgically with two-stage reconstruction using cartilage graft and paramedian forehead flap. Conclusion: Rhinophyma may occur with ventilatory difficulty and aesthetic commitment. The flap reconstruction is an option in the therapeutic arsenal. Key words: rhinophyma, plastic surgery.
