ABSTRACT Littre`s hernia is defined as the presence of a Meckel`s diverticulum in any hernial sac. We found very few reports in the medical publications due to its low incidence. There`s no clinical case reports published nationally, only references about this disease, in national textbooks like "Cirugía" of Silvio Díaz Escobar and "Cirugía. Fundamentos y Terapéutica" of Joaquín Villalba and Ricardo Morales. We report a 23 years old male that consulted in emergency unit of the Hospital de Clinicas of Facultad de Ciencias Medicas (National University of Asuncion) due to a painful irreducible tumour in the left inguinal region and he was operated with preoperative diagnosis of strangulated left inguinal hernia. We performed a left inguinal approach and a Meckel`s diverticulum was found in the hernial sac. The diverticulum was resected in wedge and the ileum was sutured. The hernia defect was repaired using Bassini technique. The patient was discharged on the eighth postoperative day in good conditions.
