Spieghel hernia (SH) is a few frequent variety of abdominal wall's hernia. Its clinical diagnosis is difficult and requires a high level of suspicion. Its treatment is always surgical. The purpose of this study is to present a case of hernia Spieghel diagnosed and treated at the Hospital IPS-Luque. The method of surgery is presented a case of hernia Spieghel clinically diagnosed and treated by open surgery in the Ambulatory Surgery program developed in our hos- pital. Is a brief bibliography. Spieghel hernias are rare and difficult to diagnose because of its intramural localization and vague and non-specific symptoms that accompany. Ultrasonography and computed tomography can help to reach the diagnosis. The high percentage of complications that make treatment is surgical, being currently in vogue the laparoscopic access. In our case we use the open technique inside the program of Out Patient Surgery, with good result. As conclusions we can mention that the HS are rare hernias of the anteriorlateral abdominal wall, mostly small, what difficult its diagnosis, with a high percentage of complications because of the given size and rigidity of its hole. The diagnosis can be clarified with the help of the ultrasonography and the computed tomography. The treatment is surgical with multiple choices, any of which can be included inside an outpatient program, in the absent of complications.
