The organizational climate is considered a factor that reflects the eases or obstacles found by the worker in the working environment. It is important to determine the employee satisfaction level in an organization to make effective interventions to improve working conditions if necessary and optimize productivity. Very few studies in Paraguay have characterized the organizational climate of a hospital and none of a military hospital. Thus, this study was conducted to determine the organizational climate with its various dimensions, perceived by medical, nursing, paramedical and administrative staff of the Central Hospital of the Armed Forces of Paraguay in 2012. This observational cross-sectional descriptive study with analytical component included a sample of 226 workers of the hospital. The data collection was performed by a self administered survey, using the PAHO "Organizational climate inventory" tool, which consists of 80 items divided into four dimensions, motivation, leadership, participation and reciprocity. Additionally, socio-demographic and labor data from the personnel were recorded. Fifty five percent of the personnel claimed dissatisfaction in the motivation dimension, 53% in reciprocity and 32% in participation as well as a high percentage in the dissatisfaction risk in leadership (83%) and participation (68%). Unsatisfactory levels were obtained in all studied dimensions related to organizational climate on the staff of the Military Hospital. It is urgent to take some pertinent interventions in order to change the employee perception of the hospital organization.
