In recent years, the tobacco consumption has constituted an important problem of public health, being one of the main causes of disease and preventable death. The objective of this work was to describe the mortality attributable to tobacco consumption in Paraguay during 1998, 1999 and 2000. Mortality by sex and cause in the age group of 35 to 65 years was obtained from the Department of Statistics of the Ministry of Public Health and Social Welfare. The percentages of non-smokers, ex-smokers and smokers were obtained from the National Home Census of 2003 made by the Anti-Drugs National Secretariat and the relative risks of death were obtained from the Cancer Prevention Study II carried out in the United States de America. The proportion of attributable deaths was calculated per year and sex for the age group selected from the proportional population attributable risk. During the studied periods, it was possible to attribute 13.4% of deaths to tobacco consumption in the 35-65 years group. Mortality was higher in men that in women and cardiovascular diseases were the most important diagnostic categories. Every day two people die in Paraguay because of diseases related to tobacco consumption, then public policies to reduce tobacco consumption must be firm and immediately developed.
