Early childhood caries is a severe form of caries present in infants and young children, which initially affects the upper teeth. The lower teeth are usually protected by the tongue, but sometimes the caries process is so severe that lower teeth are also affected. Our objective was to present a clinical case and review the literature on early childhood caries, concepts, prevalence, prevention, risk factors and treatment. We present the case of a 6 year-old girl, with severe caries and a plaque index of 100% noted in her three previous dental controls reason and for which the treatment performed had been unsuccessful due to the patient's behavior and by the lack of collaboration on the mother’s part. For this patient’s dental rehabilitation, the technique used was atraumatic restorative practice, as it is the most effective and is well tolerated by the pediatric patient. This technique takes advantage of the benefits provided by the glass ionomer, with the subsequent installation of a maintenance plate of active space, taking into account that it was necessary to perform premature extractions. At the end of the procedure, the mother was again instructed in continuing the application of preventive oral health measures in the home. The patient was referred to maxillary functional orthopedics to solve her class III skeletal problem.
