Nutcracker syndrome is uncommon and is caused by compression of the left renal vein (VRI) between the abdominal aorta and the superior mesenteric artery (anterior Nutcracker syndrome), or by compression of the VRI between the abdominal aorta and the spine, when the vein is retro-aortic (posterior Nutcracker syndrome). The syndrome can be both anterior and posterior when there is a duplication of the VRI, whose anterior tributary vein is compressed between the aorta and the superior mesenteric artery and the posterior tributary vein is compressed between the aorta and the vertebral column. Clinically, it is characterized by lumbar, pelvic or gluteal pain, micro or macrohematuria, orthostatic proteinuria and left varicocele, especially in patients with a decreased body mass index.The purpose of this report is to present the clinical case of a teenager with Nutcracker Syndrome, which presented with microhematuria and orthostatic proteinuria. As this diagnosis is infrequent, it requires a high index of suspicion.
