Introduction: Previous studies in Paraguay have shown that tooth decay rates in 5-year-old children is very high, which has led to a focus on prevention in oral health programs for preschoolers, when teething may co-exist with serious oral diseases. Objective:  To compare the prevalence of caries in children aged 1-5 using the ICDAS II, ceo-s and ceo-d indexes in the San Miguel Health Clinic in San Lorenzo from May to October, 2012. Methods: We performed a descriptive, observational, cross-sectional study with an analytical component using a non-probability sampling of consecutive cases. We included 270 boys and girls aged 1-5 years who came for routine check-ups and vaccinations, whose parents gave their consent for participation in the study. Results: We found a higher prevalence of caries by ICDAS II index in all age groups, 34.8% in 2 year-old children with an average of 2.7 ± 4.9 of tooth surfaces affected, 67.4% in the 3 year-old group with an average of 8 ± 10.5, 84.5% at 4 year-old children with an average of 12 ± 13.6 and 87% 5 year-olds with an average of 13.3 ± 12.2 . Extensive caries damage (L6) was prevalent in 23.4%. Conclusion: We found an association between age and ICDAS indexes, ceo-d and ceo-s (p <0.01) indexes. Caries prevalence increased significantly with age.
