Pyoderma gangrenosum is an uncommon recurring ulcerative skin disease of uncertain etiology that in 50% to 70% of cases is associated with systemic disease. It is even more unusual in pediatric patients. We present 3 cases of pyoderma gangrenosum, constituting 18% of all cases seen in a period of 17 years at the dermatology department of the Hospital Nacional in Itaguá, Paraguay. Of these three patients, 2 presented the ulcerative form, 1 had associated intestinal inflammatory disease, 2 were male, and all 3 presented lesions of the legs.
