IRAMAIN, Ricardo et al. Treatment of Dengue Shock in a Pediatric Emergency Unit. Pediatr. (Asunción) [online]. 2013, vol.40, n.1, pp.11-18. ISSN 1683-9803.
Introduction: Infection by the dengue virus results in syndromes that vary in severity and prognosis, ranging from dengue fever to hemorrhagic dengue and dengue shock, with this last being the most severe. Objective: To assess the effectiveness and efficacy of a treatment protocol for compensated dengue shock. Materials and Methods: We conducted a prospective, observational, and descriptive study. We included 130 patients aged 0 to 17 years with early clinical signs of shock admitted to the emergency unit between February 1, 2011 and May 30, 2012. Results: Females predominated at a ratio of 1.6:1. Clinical data at the end of the study showed significant reduction in hematocrit (p=0.007), respiratory rate (p=0.00001), and heart rate (p=0.00002).No significant difference was seen between baseline and final values for platelet count or blood pressure (p<0.05). The number of fluid boluses administered varied from 1 to 2 in 66.3% of cases. Improvement was noted in 91.6% of cases; of the 8 patients whose condition worsened, 6 required intensive care. Conclusions: The protocol used is efficacious and safe for the treatment of compensated shock
