Introduction: Varied clinical and laboratory manifestations of dengue have been described, but little has been published on dengue in patients under age 5. Physicians dealing with this disease should be aware of its less common manifestations. Objectives: To assess typical and atypical clinical and laboratory characteristics in dengue patients with compensated shock. Materials and Methods: We prospectively enrolled 155 hospitalized patients. Results: Of the 155 patients, 51 were under age 5 (UA5) and 104 over age 5 (OA5). We found irritability in 31 (60.8%) of UA5 patients vs. 4 (3.8%) OA5 patients, p= 0.001; abdominal pain in 14 (27.5%) UA5 vs. 37 (35.6%) OA5, p= 0.312; cutaneous rash in 17 (33.3%) UA5 vs. 91 (87.5%) OA5, p= 0.001; hemorrhage in 11 (21.6%) UA5 vs. 22 (21.2%) OA5, p= 0.9; diarrhea in 19 (37.3%) UA5 vs. 2 (1.9%) OA5, p= 0.001; vomiting in 20 (39.2%) UA5 vs. 27 (26.0%) OA5, p= 0.092; hepatic enzymes in 29 (57%) UA5 vs. 16 (15%) OA5, p= 0.001; HCT at admission was 36.5% UA5 vs. 39.9% for OA5, p= 0.000; baseline platelets 215,576/m3 UA5 vs. 171,096/m3 OA5, p= 0.019; final HCT was 33.3% for UA5 vs. 36.9% for OA5, p= 0.001; final platelets 155,122/m3 UA5 vs. 159,543/m3 OA5, p= 0.791. Conclusions: Our results indicate that in addition to the typical clinical manifestations of dengue, other unusual manifestations exist which may not be noted if not detected early.  Some of the laboratory data also showed results that were limited to the under 5 age group.
