Introduction: Myositis is an uncommon presentation in dengue. Case report: A 10-year old boy with fever of 4-days duration reported intense pain in the right calf accompanied by functional impotency and swelling. Laboratory tests: Blood count showed leucopenia of 3000/mm3, with a thrombocytopenia nadir of 136,000/mm3. Vascular occlusion was excluded by Doppler ultrasound. AST of 149 mU/mL, ALT 41 U/mL, CK 3775 U/L (range up to 174), serology for dengue at day 6 was IgG 5.3 and IgM 4.2.Adenovirus and influenza A or B were excluded. Treatment for symptoms was administered, and progression was favorable with complete recovery in 10 days. Discussion: The dengue virus affects a number of organs and systems. In the muscle fiber it produces inflammation characterized by mononuclear infiltration and accumulation of lipids. In the first case series published (n=40), in 2005, most patients were males with an average age of 5 years and all outcomes were good. As dengue is endemic to Paraguay, physicians in that country should be alert to the possibility of uncommon clinical presentations and at early ages. Myositis as a primary symptom is one of these presentations and diagnosis is based on the clinical presentation and elevated CK levels.
