Although in the immunocompetent host Zika virus infection is usually a mild and self-limiting disease, an increase in cases of neurological syndromes, notably Guillain-Barré syndrome, was observed during the Zika epidemic in French Polynesia in 2013 and later in different countries Such as Brazil and Colombia. We present the first case of neurological syndrome associated with Zika of postnatal acquisition in Paraguay.

It is an infant female, 9 months old, from the city of Coronel Oviedo, Paraguay, who, after a fever of 6 days of evolution, manifests symptomatology compatible with encephalitis, with progressive alteration of the sensorium that merits admission to mechanical ventilation, with non-pleocytosis CSF, and an electroencephalogram - supported convulsive syndrome. The following clinic was characterized by Guillain Barré type polyradiculoneuropathy, with weak limbs, arreflexia and CSF with cytological albumin dissociation. The electromyography was compatible with the axonal variant of Guillain Barré syndrome. Nuclear magnetic resonance was normal. The patient received immunoglobulin at 2gr/kp, with complete progressive improvement of the motility and strength of lower limbs, as well as the osteotendinous reflexes.

Given the progression and distribution of the Zika virus, especially now in the Americas, and its explosive nature in relation to the attack rate, it is possible to consider its presence as noxa in atypical neurological manifestations in patients of different ages
