ABSTRACT Introduction: One of the most disabling chronic complications in diabetics patient is the lower limb amputation. Objective: To estimate the total direct costs of surgical procedures for lower limb amputations caused by diabetes in patients taken care of at the Emergency Department of the Hospital de Clínicas in Asunción, Paraguay in 2008. Material and Methods: This is a calculation and estimation of the expenses incurred by patients, families and the Public Health Service in cases of lower limb amputations, using data from patients, who have received this treatment in 2008, and collected from the files of the Emergency Department and the Operating Room of the Hospital de Clínicas in Asunción, Paraguay. Results: 65 lower limb amputation were registered because of diabetes, distributed as follows: 29.2% toe amputation, 14% Trans Metatarsal Amputation, 30.7% Trans Fibular Amputation, 26.1% Trans Femoral Amputation. The total direct cost for each type of amputation respectively was: 11,641.61; USD 8,314.69; 18,477.09; 15,705.53 American Dollars. The total cost for all amputations was: 54,138.93 American Dollars.
