The hiatal hernia is an anatomic condition acquired; it defines as the protrusion or the prolapsed of the proximal stomach or any other non esophagic structure, through the esophageal hiatus. The giant hiatal hernia with intrathoracic stomach is an infrequent disease. It corresponds to a type III hernia or mixed hernia (sliding and paraesophageal components) that contains more than 30% of the stomach in the thorax. This case is of a patient, 68 years old, feminine, with an image of gian hiatal hernia on an upper GI, it was performed a laparoscopic Nissen surgery. This is a revision about the pathology, its incidence and it`s therapeutic and diagnosis options.
