Abstract

The repair of soft tissue injuries of the distal third of the lower limb is a challenge for reconstructive plastic surgery. The objective of this study is to illustrate one of the indications of the sural flap for the adequate coverage of a complex wound in the calcaneal region. We describe the case of a 6-year-old boy with a motorcycle accident history, which caused a wound with loss of substance in the right calcaneal region, with small bone exposure. Treated initially with healing and then installation of aspiration system. Surgical Technique: Flap design, dissection of the skin island including the fascia, identification of the proximal vascular-nervous package, ligature and section, fasciocutaneous dissection from proximal to distal. Mobilization of the flap and coverage of the defect. Partial skin graft in the donor site. Favorable evolution. The sural flap allows reliable skin coverage of the distal third of the leg and foot when the use of grafts is not feasible. Advantages: it does not depend on microsurgery, only one operative time, without significant functional deficit, wide range of rotation and acceptable morbidity of the donor site. Disadvantage: Sacrifice of the sural nerve. It is concluded that the sural flap allows the reconstruction of defects in the distal third of the leg.
